The saddest aspect is that neither the Railway Safety & Standards Board (RSSB) nor CIRAS were able to detect the signs that this was an issue.

Perhaps it is time that all these ‘safety’ related organizations were rolled into one? Better still, perhaps Network Rail can do it better and thus save the industry millions on these redundant companies.
